I realized that the 401k deduction is "unclicked" for all taxes and should not be. All of the boxes are grayed out and not able to be changed. How do I change what boxes are checked for 401k deduction?
Neuse Regional Library-
Since deduction settings can effect how payroll tax reports are calculated, once a deduction has been used during the current year it cannot be edited and the taxable options are greyed out. If you need to make changes to your deduction, please review Knowledge Base article #18753 "How to change the taxable status or Tax Category of a deduction or other pay" for your options.

Where do I find the Knowledge base articles?
To access the Knowledge Base go to support.na.sage.com OR from Sage City, go to the Menu option "Resources" above. Then in the Assistance section, select “Technical Support Articles”
In the box where it says “Search the Knowledgebase” enter the article or topic you are searching. In this case you can enter the article #18753
