2015 with and without SR1 installed - Access Violation TSOLAddIn.DLL

We uninstalled TS, renamed and deleted all the folders on the local workstations and server - after making a backup. I installed TS 2015 on the server, installed it on the workstations. The TS add-in for Outlook shows in the Outlook 2007 menu. You can tag times, you can convert to slips.

All users aget an "Access violation at address 07F81D67 in moudule 'TSOLAddIn.DLL" read of address 00000000.

If they click OK they can sometimes complete tagging the time, but the Completed Meetings/Appointments items disappear - you no longer show them in the list of found completed items.

As a side note - if you tag an appointment that has a future date/time or one that hasn't exceeded the ending date/time, it does not appear in the Completed Meetings/Appointments even though you've marked it as PENDING.

I applied SR 1 to the server and workstations - the problem has not changed.

If I delete ALL appointments/meetings in outlook, it does not create the access violation. If I create any past, current, future meeting/appointment it will generate the access violation.

6 workstations, even on that was just built, have this problem. I see complaints about access violations, but no solutions.

Any help is appreciated.
