Third Party Disability Pay

We have a third party disability insurance company that is acting as the employer with respect to short term disability payments to our employees.  They are acting as the employer with regard to these payments in the following:  They withhold and remit all FICA taxes (employee and employer) under their FEIN and are issuing the W-2 for this pay under their FEIN.  The new agreement we have with this company transfers responsibility for FUTA/SUTA taxes to us.  How do we get this pay in enhanced tax reporting in order for it to show up on the state unemployment returns and the Federal 940.  It will not be on our 941's as we are not issuing the W-2.
