Strange Issue after upgrade

Ever since upgrading from Mas90 vers 4.05 to vers 4.5 and beyond, a specific customers name appears next to the Ship to Addr field (see image).  As soon as you select the next number in Sales Order Entry it appears, and stays there unless the customer number you select has a Default Ship to Address.  It doesn't print on anything and doesn't seem to cause any issues.

I thought it might go away when I upgraded to Sage100 2014, but it still appears.  Has anyone seen this, and should I be concerned that it's happening?

    Subject:

    In Sales Order Entry, when clicking the Next Order No. button, an invalid description displays next to the Ship to Address field until a customer is selected.

    Possible Resolution:

    This issue may occur if the SO_ShipToAddress.M4T file contains an invalid record. The invalid record may be removed by using Data File Display and Maintenance.

    Note: Use caution when working with Data File Display and Maintenance. Always make a backup first. If necessary, seek the assistance of a qualified Sage Software business partner or Sage Software Customer Support analyst.

    On the Tasks tab, expand Modules, Library Master, and Utilities. Double-click Data File Display and Maintenance.

    A warning message displays; read the message, and then click OK.

    Browse to ..\Mas90\Mas_xxx\SOxxx\SO_ShipToAddress.m4t (xxx  = the company code). Click SO_ShipToAddress.m4t, and then click the OK button. The first record in this file displays.

    The key should display as follows: Key=[01CustomerCode.1]

    If an invalid record exists, the key may display as follows: Key=[.]

    Look at field number 4, ShipToName. If the description is the same one that incorrectly displays in Sales Order Entry, the record may be deleted.

    While the invalid record displays, click the Delete button. Click Yes when "Delete the record with the following key? 01CustomerCodexxxx1" displays.

    Close Data File Display and Maintenance.

    It was in the archive knowledgebase - couldn't find it in the 'new' knowledgebase.. Unfortunately we're going to lose the archive soon, for some reason..

    I should have addressed it sooner anyway, not sure how many years it's been since I upgraded from 4.05 to 4.5, which is when it first appeared.
